In order to save you time on your visit and help us to stay on time for your appointment, please take a few moments now to gather the items you will need and register or update your records using the secure patient portal for the digital medical record system.
If you have not completed the patient portal digital chart prior to your appointment, it is critical to arrive 30 minutes early as your medical information needs to be entered in our system prior to your visit. Optimal use of your time spent with your Provider is our goal.
There will be additional forms that we will ask you to complete when you arrive for your appointment. Please bring the following:
- Current insurance card
- List of current medications (including vitamins and supplements)
- Physician referral if needed
- Copies of any pertinent diagnostic tests or medical records
